Greetings to the SRFI 134 list and to the SRFI editor.

While preparing the SRFI 134 sample implementation for packaging
under CHICKEN, I noticed that it is attributed (in a header comment)
to Shiro Kawai and licensed under a 3-clause BSD license.  The SRFI
document reads "The two-list implementation was written by John Cowan",
and does not mention any contributions by Shiro.

An erratum may be in order to correct the attribution of the sample
implementation.  The licensing is also a point of issue; in my CHICKEN
package, I've included both the standard SRFI license (with Wortman &
Cowan as copyright holders) and the BSD license (attr. Shiro Kawai)
from the implementation.  This is somewhat confusing, and
clarification would be helpful.
