Aargh.  Now I do too, even though I searched through the file
multiple times.  Perhaps a combination of a cut-and-paste error
into my local copy and confusion when scanning the web version.

Also note that the PLT version in reference [5] does seem to be
missing ra:for-each, even though it has ra:map:
