I've found couple of errors on the latest draft:

- hash-table-push! is listed on Index, but there's no definition.
- hash-table-keys, hash-table-values, and hash-table-entries are listed on
  the 'The whole hash table' in Index, but the definition is written in
  Copying and conversion.

And I found the followings a bit difficult to read:

- All procedures described in 'Hash functions and reflectivity' section
  are deprecated. But the section seems rather important because it's
  right after the 'Constructor' section.
- It might be friendly to have index item 'Deprecated procedures' or
  at least listed on Index. Or are implementations allowed not to export
  these procedures?
