Thanks to Jim Rees and Per Bothner for finding and reporting
this bug so quickly, and to Marc for applying the fix so
quickly.  I've just published Marc's new draft, which
contains the correction Jim and Per suggested.  I'm
embarrassed to say that I stared at that example before
draft #1 and convinced myself that it was right.  Oops.
