How is the state variable "col" supposed to interact with "columnar"?

I would say it makes most sense when "col" is reset to "0" at the
start of each column line and for each column.

The reference implementation resets "col" to the value "col" had at
the start of the whole columnar, which doesn't make much sense (in
particular for the second, third, ... column).
