There are two changes in this version, both previously discussed here
in the mailing list. First, the editing error in the Pitfalls section,
lambda instead of stream-lambda, was fixed. Second, the implementation
of stream-cons was changed to use stream-eager instead of
stream-delay. Thanks to Jos Koot for both changes.
